ABSTRACT

Objective:To investigate the effects of dexmedetomidine on vital signs during recovery from general anesthesia in gynecological patients undergoing general anesthesia surgery and analysis of risk factors for complications.Methods:A total of 80 gynecological patients undergoing general anesthesia surgery who received treatment in Lishui People's Hospital from March 2021 to March 2022 were included in this study. They were randomly divided into an observation group and a control group ( n = 40/group). All patients were subjected to general anesthesia. The observation group was infused with 0.5 μg/kg dexmedetomidine intravenously 15 minutes before induction of anesthesia and then infused with dexmedetomidine at a rate of 0.2 μg/kg per hour until 20-30 minutes before the end of the operation. The control group was identically given 0.9% normal saline. The recovery quality, vital signs before surgery and during recovery from general anesthesia (systolic blood pressure, diastolic blood pressure, heart rate, body temperature), and complications during recovery from general anesthesia were compared between the two groups. These patients were divided into a complication group and a non-complication group according to whether there were complications during recovery from general anesthesia. Univariate and multivariate Logistic regression analyses were performed to analyze the high-risk factors for complications occurring during recovery from general anesthesia in gynecological patients undergoing general anesthesia surgery. Results:The time to awaken, time to recover spontaneous respiration, and time to extubation in the observation group were significantly shorter than those in the control group ( t = 3.74, 2.97, 2.56, all P < 0.05). Systolic blood pressure, diastolic blood pressure, and heart rate measured during recovery from general anesthesia were significantly lower in the observation group compared with the control group ( t = 5.71, 4.53, 4.53, all P < 0.001). Body temperature ( t = 4.40, P < 0.001) and the incidence of complications ( χ2 = 5.69, P < 0.05) were significantly lower in the observation group compared with the control group. These patients were divided into complication ( n = 22) and non-complication ( n = 58) groups according to whether they had complications during recovery from general anesthesia. Univariate and multivariate logistic regression analyses showed that American Association of Anesthesiologists grade II, presence of underlying diseases, abnormal leukocyte count, and no use of dexmedetomidine were the risk factors for postoperative complications in gynecological patients undergoing general anesthesia surgery ( OR = 2.38, 2.86, 2.17, 3.60, all P < 0.05). Conclusion:Dexmedetomidine can improve awakening quality and vital signs and reduce complications during recovery from general anesthesia. American Association of Anesthesiologists grade, underlying disease, abnormal white blood cell count, and no use of dexmedetomidine are the risk factors for complications occurring during recovery from general anesthesia in gynecological patients undergoing general anesthesia surgery.

ABSTRACT

The mixed evaluation system uses the network platform as a carrier to construct a dynamic evaluation model that spans time and space between online and offline, between individuals and teams. Mixed evaluation system of physiology was performed for two years, and the process evaluation and the summative evaluation in the mixed evaluation system showed a good correlation, and the students' learning ability and learning effect showed consistency with the evaluation results. Practice has proved that the mixed evaluation system not only optimizes the traditional evaluation system, but also fully expands the five principles of formative evaluation; however, while reflecting the advantages of the network platform, it also exposes the defects of insufficient supervision of the network platform. Thus, we should further improve the mixed evaluation system of physiology.

ABSTRACT

Objective To investigate the clinical application value of 18F-FDG PET-CT simulation localization in radiotherapy of recurrent abdominal and pelvic tumors. Methods 18F-FDG PET-CT was used to simulate positioning 38 patients with abdominal and pelvic tumors who relapsed after treatment.Based on both CT images and 18F-FDG PRT-CT, we drew up a systemic treatment plan and outlined the radiotherapy target area, and then compared the differences between the two methods. Results In 38 patients, 21.1%(3/8) of patients were found to have distal metastases outside the pelvic and abdominal cavity, and changed the systemic treatment plan.The radiotherapy target was altered in 34(89.5%) patients.The mean value of GTVPET-CT was 118.14cm3and the mean value of GTVCT was 148.53cm3(P=0.044). Conclusion For patients with recurrent abdominal and pelvic tumors, 18F-FDG PET-CT simulation localization treatment improves tumor re-staging, changes the integrated therapy for some patients, and makes the target area of radiotherapy more accurate.

Dietary factors and nutritional status are closely related to the occurrence of cognitive impairment in the elderly. Excessive intake of nutrients such as lipids and carbohydrates in the diet is related to the occurrence of cognitive impairment. However, certain trace elements in the diet have a preventive effect on cognitive impairment. Based on the correlation analysis of dietary factors, nutritional status and cognitive impairment in the elderly, this paper expounds the relationship between cognitive impairment and its specific mechanism of action, in order to change the lifestyle of the elderly with cognitive impairment to improve their quality of life, and to provide a certain theoretical basis for the construction of dietary habits in line with national life.

ABSTRACT

Chronic obstructive pulmonary disease (COPD) is an important healthcare problem worldwide. Often, glucocorticoid (GC) resistance develops during COPD treatment. As a classic hypoglycemic drug, metformin (MET) can be used as a treatment strategy for COPD due to its anti-inflammatory and antioxidant effects, but its specific mechanism of action is not known. We aimed to clarify the role of MET on COPD and cigarette smoke extract (CSE)-induced GC resistance. Through establishment of a COPD model in rats, we found that MET could improve lung function, reduce pathological injury, as well as reduce the level of inflammation and oxidative stress in COPD, and upregulate expression of nuclear factor E2-related factor 2 (Nrf2), heme oxygenase-1 (HO-1), multidrug resistance protein 1 (MRP1), and histone deacetylase 2 (HDAC2). By establishing a model of GC resistance in human bronchial epithelial cells stimulated by CSE, we found that MET reduced secretion of interleukin-8, and could upregulate expression of Nrf2, HO-1, MRP1, and HDAC2. MET could also increase the inhibition of MRP1 efflux by MK571 significantly, and increase expression of HDAC2 mRNA and protein. In conclusion, MET may upregulate MRP1 expression by activating the Nrf2/HO-1 signaling pathway, and then regulate expression of HDAC2 protein to reduce GC resistance.

ABSTRACT

Objective:To understand the monitoring levels of individual dose to radiation workers in Jiangsu province from 2011 to 2018, and to analyze their changing trends.Methods:Through National Individual Dose Registry, the data on the monitoring result of occupational external exposure of radiation workers in our province was collected, and statistical analysis was performed.Results:A total of 176 134 radiation workers in Jiangsu province were collected from 2011 to 2018. The annual collective effective dose was 58 man·Sv and the average annual effective dose was 0.35 mSv. The average annual effective doses to medical radiation workers for interventional radiology and nuclear medicine were both 0.41 mSv, with significantly higher NRE ratios than others ( χ2=219.1, P<0.05). By 2018, the numbers of monitored workers were 25 714 for medical radiation, accounting for 81%, with an average annual effective dose of 0.24 mSv, and 6 169 for industrial radiation, accounting for 19%, with an average annual effective dose of 0.18 mSv. Conclusions:The average annual effective dose to radiation workers in Jiangsu province is in consistency with the relevant national standards. The dose values for radiation workers in interventional and nuclear medicine are relatively high. The protection measures in their workplace should be improved. Meanwhile oversight of radiation protection should be enhanced for individual radiation workers in Jiangsu province.

OBJECTIVE@#To explore the mechanism by which ginsenoside 20(S)-Rg3 upregulates the expression of tumor suppressor von Hippel-Lindau (VHL) gene in ovarian cancer cells.@*METHODS@#Ovarian cancer cell line SKOV3 treated with 20(S)-Rg3 were examined for mRNA and protein levels of VHL, DNMT1, DNMT3A and DNMT3B by real-time PCR and Western blotting, respectively. The changes in VHL mRNA expression in SKOV3 cells in response to treatment with 5-Aza-CdR, a DNA methyltransferase inhibitor, were detected using real-time PCR. VHL gene promoter methylation was examined with methylation-specific PCR and VHL expression levels were determined with real-time PCR and Western blotting in non-treated or 20(S)-Rg3-treated SKOV3 cells and in 20(S)-Rg3-treated DNMT3A-overexpressing SKOV3 cells. VHL and DNMT3A protein levels were detected by immunohistochemistry in subcutaneous SKOV3 cell xenografts in nude mice.@*RESULTS@#Treatment of SKOV3 cells with 20(S)-Rg3 significantly upregulated VHL and downregulated DNMT3A expressions at both the mRNA and protein levels (@*CONCLUSIONS@#Ginsenoside 20(S)-Rg3 upregulates VHL expression in ovarian cancer cells by suppressing DNMT3A-mediated DNA methylation.

ABSTRACT

Objective To validate the method for measuring the TPV and OAR doses and 2D dose distribution in IMRT through using TLD and radiochromic film.Methods Eight medical linear accelerators (Valian,Elekta,Siemens) were selected.The polystyrene phantom provided by IAEA was CT scanned and the image obtained was transferred to TPS for formulation of treatment plan,prescription of PTV and OAR doses and calculation of corresponding monitoring unit (MU),IMRT was performed on the phantom using 6 MV X-ray.Irradiated TLDs and films were measured and evaluated at the Secondary Standard Dosimetry Laboratory at the Radiation Safety Institute of Chinese Center for Disease Control and Prevention.Results According to IAEA requirement,the relative deviations between TLD-measured and TPS-planned doses were within ±7.0% for the prescribed PTV and OAR doses.As measured result,the PTV values for 8 accelerators were in the range of 0.6% to 5.9%,consistent with the IAEA requirements,whereas the OAT values for 8 accelerators were within-0.6% to 7.0%,consistent the requirements.As IAEA required,the 2D dose distribution passing rate of 3 mm/3% should be higher than 90%.The filmmeasured and TPS-planned values for 8 accelerators were within 90.2% to 100.0%,consistent with the requirements.Conclusions TLD and radiochromic film are feasible in validating the PTV and OAR doses and the 2D dose distribution pass rate in IMRT.This method can be widely used in quality audit and internal verification in IMRT in medical institutiions on a large scale.

ABSTRACT

Objective:To evaluate the effectiveness and safety of early exercise safety management in patients undergoing mechanical ventilation in intensive care unit (ICU).Methods:A prospective historical control observation was conducted. Forty-five patients with severe respiratory failure undergoing mechanical ventilation admitted to the ICU of Affiliated Hospital of Jining Medical University from April to June in 2019 were enrolled in the observation group and implemented early exercise safety management, including establishing multidisciplinary safety management team with ICU doctors, ICU nurses, respiratory therapists, rehabilitation therapists, dietitians and psychological consultants to jointly develop early exercise plan; equipping with sports and safety protection equipment; assessing the early exercise risks, formulating early exercise prescriptions; formulating the nutritional and psychological prescriptions; carrying out the propaganda and education in the early exercise process of patients and communicating with patients timely; strengthening team training. Other 45 patients with severe respiratory failure admitted for mechanical ventilation from January to March in 2019 were enrolled in the control group, whose gender, age and artificial airway conditions were matched with those in the observation group. Routine mechanical ventilation nursing was performed, including condition monitoring, medication nursing, environmental management and routine examination assistance. The improvement of depression, anxiety, comfort, lung function, and quality of life before and after nursing intervention were observed, and the occurrence of complications was recorded.Results:There was no significant difference in gender, age or artificial airway conditions between the two groups. Before nursing intervention, there was no significant difference in depression, anxiety, comfort, lung function and quality of life between the two groups. After the intervention of different nursing programs, the above conditions of the two groups were significantly improved, and the improvement was more significant in the observation group, which showed that the hospital anxiety and depression scale (HAD) score and forced vital capacity (FVC) of the observation group were significantly lower than those of the control group [HAD score: 10.80±2.54 vs. 17.51±3.66, FVC (L): 1.81±0.42 vs. 2.23±0.39, both P < 0.01], while the general comfort questionnaire (GCQ) score, forced expiratory volume in 1 second (FEV1), FEV1/FVC ratio, FEV1 percentage of predicted (FEV1%) and each dimension score of 36-item short form health survey (SF-36) scale were significantly higher than those of the control group [GCQ score: 110.87±5.33 vs. 96.93±3.02, FEV1 (L): 1.99±0.37 vs. 1.71±0.15, FEV1/FVC ratio: 0.88±0.04 vs. 0.84±0.03, FEV1%: (88.98±8.57)% vs. (80.41±4.45)%, mental function score: 49.74±9.88 vs. 40.17±8.95, physical function score: 27.65±9.46 vs. 20.32±9.53, social relationship score: 62.14±6.33 vs. 50.28±8.76, general health score: 38.61±8.48 vs. 30.63±8.93, all P < 0.01]. The overall incidence of complications in the observation group was significantly lower than that in the control group (24.44% vs. 57.78%, P < 0.01). Conclusions:Early exercise safety management scheme for patients undergoing mechanical ventilation in ICU can improve clinical efficacy and reduce complications. At the same time, it can further standardize the behaviors of medical staff and ensure the early exercise.

Objective: To explore the differences of wear resistance of three kinds of glass ceramics and Wieland Zenostar zircona (Zenostar). and to clarify their influencing factors. Methods: Zenostar were made into flat-shaped specimens (zirconia base sample group) and hemisphere-shaped specimens (zirconia pair grinding group). There kinds of glass ceramics IPS Empress (Empress). IPS e. max CAD (e. max). VITA Suprinity (Suprinty) were used as base specimens. Each group was exposed to UMT-2 testing machine to simulate the clinical service. The wear depthes of base specimens were detected by laser confocal scanning. Scanning electron microscope (SEND was used to evaluate the wear surfaces. Results: In zirconia base sample group, there were no significant differences in the maximum wear depthes to Zenostar between the three kinds of glass ceramics ( P>0. 05). ABSTRACT

Inflammation promotes tumor initiate,progress,invade and metastasize.Hematological inflammation biomarkers in nasopharyngeal carcinoma (peripheral blood neurtrophil-to-lymphocyte ratio,plateletto-lymphocyte ratio,systemic immune-inflammation index and C-creative protein) are the expressions of inflammation in the progress of nasopharyngeal carcinoma.The information of prognosis can be obtained by testing the hematological inflammation biomarkers in nasopharyngeal carcinoma patients.These biomarkers are the supplement for the current prognosis predictive indices and they can be used to guide the clinical practice,which will provide more ideas.

ABSTRACT

Objective To measure the accuracy of multi-leaves collimator (MLC) leaves positions in intensity modulated radiation therapy (IMRT) for verification purpose.Methods Solid water homogeneous phantom with size of 30 cm× 30 cm was scanned by CT scanner.The scanned images were delivered to radiation therapy plan system (TPS) to formulate the therapy plan.The MLC leaves created 5 strips of exposure field,each 3 cm long and 0.6 cm wide.The strip-to-strip distance was 3 cm.With 6 MV X-rays,the SSD was 100 cm at the maximum dose point.The MU per strip was 250 MU.EBT2 radiochromic films each of 25 cm×25 cm were placed on the 30 cm×30 cm homogeneous solid phantom and covered with 1 cm thick solid phantom slabs for delivering of IMRT.Results For 7 of 8 accelerators,the differences of film-measured and TPS-planned MLC leaf position for every fence field were within ± 0.5 mm as required by IAEA,with only other one being-0.6 mm,not consistent with the IAEA requirements.The film-measured position difference between each pair and all pairs of leaves for 8 accelerators were all in line with IAEA's requirements.The film-measured actual width difference between each pair and all pairs of leaves was within ±0.75 mm as required by IAEA for 4 accelerators and outside ±0.75 mm for the other three,not consistent with the IAEA requirements.The standard deviation of film-measured actual width between each pair and all pairs of leaves for 6 accelerators were ≤ 0.3 mm,as required by IAEA,whereas >0.3 mm for the other two,not consistent with IAEA requirements.Conclusions The film dosimetric verification of IMRT is an important part of its quality assurance,featuring simple,reliable and highly accurate positioning and can meet measurement requirement.

Objective@#To evaluate the safety, feasibility and operational effectiveness of master-slave system controlled Robot-assisted flexible endoscopy 'YunSRobot’ in upper gastrointestinal endoscopy examination in living pigs. @*Methods@#Seven Guangxi BA-MA Mini-pigs underwent upper gastrointestinal endoscopy examination with master-slave system controlled Robot-assisted flexible endoscopy 'YunSRobot’ and Olympus GIF-Q260J gastroendoscope (Japan). The experiment divided into manual operation group and robot operation group. Nine gastroenterologists skilled with endoscopy operation handled the robot once in vitro to familiar with the mechanism and method of robot operation. And then performed manual operation and robot operation four times each. Endoscopy pictures and operation time were recorded. T test and one-way analysis of variance were performed for statistical analysis. @*Results@#Both manual operation and robot operation could successfully finish standard esophagus and stomach endoscopic examination. There were no complications such as hemorrhage, perforation, mistaken insertion into of trachea, injures of pharynx and larynx and inhalation pneumonia. Esophagus, gastric cardia, gastric fundus, gastric body, gastric angle, gastric antrum and gastric pylorus could be clearly observed in both groups. The manual operating time of nine gastroenterologists was (3.67±1.95) minutes, which was shorter than that of robot operating time ((7.60±2.00) minutes), and the difference was statistically significant (t=8.445, P<0.01). The time of first in vitro operation performed by nine gastroenterologist was (13.10±6.32) minutes. The operation times of each case of nine gastrienterologist were (8.49±0.90) minutes, (7.50±1.19) minutes, (7.30±1.33) minutes and (7.12±1.61) minutes. The difference was statistically significant (F=7.901, P<0.01). Operation time gradually shortened and operation proficiency significantly improved. @*Conclusion@#Master-slave system controlled Robot-assisted flexible endoscopy 'YunSRobot’ which provides a clear observation of esophagus and stomach is safe in endoscopic examination of living animal.

ABSTRACT

Objective To evaluate the feasibility and safety of the robot - assisted system YunSRobot for remote manipulation endoscopy. Methods When the master of YunSRobot was installed in the gastroenterology office in Chinese PLA General Hospital, the robot slave and upper gastrointestinal simulation model (Takahashi Lm103,Japan) were installed at the same time in the State Key Laboratory of Robotics, Shenyang Institute of Automation. Three physicians were trained to operate the master robotics and performed gastroscopy on the simulation model based on network cloud. Each physician performed 3 procedures of oesophagogastroduodenoscopy (EGD) by YunSRobot using traditional manual endoscopy, on-site operating mode, and remote manipulation mode, respectively. The operating time, lumenal anatomic exposure,man-machine interaction and other parameters were recorded. Results The number of standard pictures obtained by traditional manual endoscopy group, on-site operating group and remote manipulation group were 39.9±0.3, 39.8±0.4, 39.9±0.3, respectively. The images of all five lesions could be obtained by each operation. The operating time in the duodenum of remote group was longer than that of on-site group, with average time (78.2±16.0)s vs. (68.9±15.8)s (P=0.021) respectively. As to the operating time on other parts or total time, all three groups were comparable. Although there was a mean delay of (572.1±48.5) ms in remote operation group, the operation was still smooth. However, compared with on-site group, the percentage of clear view time in the duodenum was significantly shortened in remote group: [(77.8±8.2)% vs. (83.9 ± 6.4)% , P=0.024]. Statistically significant difference was detected in percentage of clear view time neither in other sites, nor was in the total operating time between two groups. The operating time in each part of remote group was obviously longer than that of manual group as followings, pharyngeal (27.3±4.2) s vs. (9.2±1.3)s (P<0.001), esophageal (29.7±6.4)s vs. (19.3±1.6)s (P=0.004), stomach (56.7±17.0)s vs. (40.3±7.0)s (P=0.003), pylorus (20.2±5.5)s vs. (9.3±1.3)s (P<0.001), duodenum (78.2±16.0)s vs. (29.3±5.6)s (P<0.001). Thus the total operating time was also longer in remote group as (559.0±87.2)s vs. (253.1±16.6)s (P<0.001). The respective time in pharynx, esophagus, stomach, pylorus, duodenum, or the overall time was all longer in remote group than that in manual group. Conclusions The soft endoscopy robot YunSRobot has satisfactory safety and stability. Remote upper gastrointestinal endoscopy can be completed based on common network and an endoscope simulation model with smooth operation. The inspection time by YunSRobot robot per part and the overall time are longer than those of manual operation on site, still, remote operating time meets the standard of upper gastrointestinal endoscopy.

ABSTRACT

Objective To explore the dosimetric verification method for absorbed dose and twodimensional dose distribution generated through multi-leaf collimator in radiotherapy.Methods A total of 8 medical accelerators were selected in Jiangsu province.Under 6 MV X-rays with a 5 cm × 5 cm field,the thermoluminescent dosimeter (TLD) and EBT3 radiochromic film (RCF) were used to audit the values calculated by treatment planning system (TPS).RCF was used to verify two-dimensional dose distribution under the 6 MV MLC beams for intensity modulated radiation therapy (IMRT).Results The relative dose deviation was from-1.4% to 6.8% for TLD and from-1.8% to 7.8% for RCF,with 7 accelerators meeting with the IAEA deviation requirement of ± 5%.The passing rates of two-dimensional dose distribution were between 73.7% and 97.0%,with 7 accelerators greater than 90%.Conclusions TLD and RCF are feasible and suitable for large-scale verification of IMRT TPS absorbed dose and twodimensional dose distribution.

ABSTRACT

Objective:To observe the development of dentinal microcracks after root canal preparation with there kinds of reciprocating nickel-titaninm instruments using an in situ cadaver model by means of micro-computed tomography (Micro CT),and to provide the reference for their clinical application.Methods:A total of 15 mandible bone specimens having at least the anterior teeth (n=60) with single root were selected and there was no dentina microcrack,then they were randomly divided into 4 groups (n=15) according to the preparation protocol:Wave One,Reciproc,One File and ProTaper Universal groups,and ProTaper Universal was used as control group.The root canals were prepared up to 25 # instruments in Wave One,Reciproc and One File groups and F2 instruments in ProTaper Universal groups.After the preparation procedures,the specimens were scanned by Micro CT again,and the number of dentinal microcracks were investigated and analyzed statistically.Results:Compared with ProTaper Universal group,the incidence rates in Wave One,Reciproc and One File groups were significantly reduced (P<0.05).There were no statistical differences in the incidence rates of dentinal microcracks between Wave One,Reciproc and One File groups (P>0.05).Conclusion:Root canal preparation can cause the dentinal microcracks,so it is important to choose the types of instruments.Root canals prepared with Reciproc,Wave One and One File produce less dentinal microcracks,which can decrease the risk of vertical root fracture.

ABSTRACT

Objective To explore the memory function and the effects of sleep on memory consolidation in patients with stable paranoid schizophrenia.Method Sixty patients with stable paranoid schizophrenia were randomly assigned into research group (n =30) and control group (n =30).30 healthy people were raised as a healthy control group.The polysomnography (PSG) was used for 10 hours of sleep monitoring in the research group and the healthy control group and the muhiple memory assessment scale(MMAS) was used to test the subjects memory before and after the sleep monitoring.Control group performed memory tests in equal length daytime.Pre-test and post-memory measurements were compared.Results The prememory test of three groups had significant statistical differences (F(2.87) =31.40,39.89;P<0.01).Both the auditory verbal/picture memory conservation rate and recognition accuracy rate between research group and healthy controls had no statistically significance differences (P>0.05).But compared with the control group,auditory verbal memory conservation rate ((62.64±9.06) % vs (43.00±9.73) %,P<0.01),recognition accuracy rate ((69.11±11.04)% vs (61.78±11.67)%,P<0.05),picture memory conservation rate((71.20±14.95) % vs (58.72 ± 16.19) %,P< 0.01),recognition accuracy rate ((86.89 ± 8.02) % vs (78.89 ±12.63) %,P<0.01) of the research group showed statistical significance differences (P<0.05).Both N2 (r=0.377,P<0.05) and REM (r=0.436,P<0.05) had positive correlation with recognition accuracy rate of auditory verbal memory in research group.Conclusion Stable paranoid schizophrenic patients have poorer memory function(auditory verbal words memory recall,picture memory recall) than healthy people.Sleep promotes the consolidation of memory in patients with schizophrenia.Both N2 and REM of patients with stable paranoid schizophrenic have positive correlation with recognition accuracy rate of auditory verbal memory.

ABSTRACT

Objective To improve the in vivo analgesic activity of acacetin and find leads for the development of new drugs, novel acacetin derivatives containing alkyl amide groups with different length of carbon chain were designed and synthesized according to the molecular structure of the active hit compound found in our previous work. Methods Using apigenin as the initial chemical ma-terial,the acacetin was synthesized through 3 steps,then the target compounds were prepared by conjugating hydroxy group of acace-tin at position 7 with bromoalkyl amides. The analgesic activity of the target compounds was evaluated by acetic acid writhing model of mice. Results and Conclusion Novel acacetin alkyl amide derivatives showed more potent analgesic activities than that of clinical medicine diclofenac,which could be used as leads for further development of new drugs.
